Sainsbury Management Fellowships in the Life Sciences: Instructions to Applicants

Please provide evidence of all qualifications including membership of professional bodies (photocopies will be sufficient).

1. Complete the application form with as much detail as possible (including duration of proposed course and dates if possible).

2. The following information should accompany the completed application form when it is submitted.

   • Four passport-sized photographs,

   • An up-to-date curriculum vitae,

   • Photocopies of certificates relating to academic qualifications and membership of professional bodies,

   • A Personal Development Plan.

3. The Personal Development Plan is a list of activities the applicant proposes to undertake to achieve their career goals. Activities should be specified with as much detail as possible.
